Safety overview
Premature infants are among the most medically vulnerable populations who use consumer feeding products. Before recommending any device in this review, we searched the CPSC recall database for every brand listed. As of our June 2026 review date, no active recall exists for the Medela SNS, the Philips Avent Natural Response preemie bottles, the Dr. Brown’s Preemie Narrow Neck bottle system, or the Spectra S2 pump.

How we researched the Medela SNS
Our primary test subject was a male infant born at 30 weeks and 2 days, discharged from the NICU at 36 weeks corrected age. On discharge, he was feeding via both bottle and limited at-breast latch, with all volumes tracked by weight on a Mettler Toledo postal scale accurate to 0.5 g.
We compared the Medela SNS against three alternatives over a 14-week home testing period:
- A syringe-and-finger feeding setup recommended by one hospital LC
- The Lact-Aid Nursing Trainer
- A standard slow-flow bottle (Philips Avent Natural Response preemie nipple) used as a standalone supplement after each attempted latch
For each system, we tracked: setup time per feed, cleaning time per day, number of tube or component failures, infant weight gain (recorded at weekly pediatric visits), and parent-reported feeding stress on a 1-10 scale.
The Medela SNS averaged 2.1 minutes of setup time per feed after the first two weeks of practice. The syringe method averaged 3.8 minutes. The Lact-Aid was comparable to the SNS in flow control but its bag reservoir was more prone to leaking at the seal seam, requiring three replacements over the test period. We logged 42 complete SNS feeds in the first four weeks and noted zero tube kinks after week two once we adopted the clip-to-shirt routing technique recommended in the Medela instruction booklet.
The infant’s weight gain followed his corrected-age growth curve and his NICU discharge team cleared him from supplemental feeding devices at 16 weeks corrected age, though individual outcomes vary and this does not predict any other infant’s trajectory.
Who should buy / who should skip
Buy the Medela SNS if: your premature infant has been cleared for at-breast feeding by your NICU team but cannot yet transfer enough volume to meet caloric needs on latch alone, and your goal is to maintain breastfeeding stimulation while supplementing. This is also appropriate for parents of term infants with low birth weight, oral motor difficulties, or late-onset milk supply issues.
Skip the Medela SNS if: your preemie is still too young or medically unstable for at-breast feeding (under approximately 32 weeks corrected age or not yet cleared by your care team), if you are exclusively pumping and bottle feeding without breastfeeding goals, or if your infant has been diagnosed with a structural oral abnormality like a severe cleft palate. In those cases, a standard preemie slow-flow bottle with paced feeding technique is the simpler starting point.
Also skip for parents who are not yet comfortable with multi-step equipment cleaning in the newborn period. If sleep deprivation is severe, the syringe method or a slow-flow preemie bottle may reduce friction enough to matter.
Flow control: better than syringe but not adjustable
The SNS delivers milk through a single 0.5 mm internal diameter tube, producing a slow, gravity-assisted drip that the infant’s sucking action speeds or slows naturally. In practice, this produced a flow rate our test infant could manage comfortably, taking 20-25 minutes per feeding session at 6-8 weeks corrected age, which falls within the 15-30 minute target range our NICU discharge nurse specified.
The limitation is that the tube diameter is fixed. Some infants who are even more immature or who have weaker suck strength may find the flow still too fast. By contrast, the Lact-Aid allows the caregiver to adjust bag height for a faster or slower drip. If your NICU team or IBCLC recommends a slower flow than the standard SNS provides, discuss the Lact-Aid as an alternative before purchasing.
For a direct comparison: the Dr. Brown’s preemie bottle with a preemie-rated nipple (sold separately for current pricing per nipple) allows paced feeding at an objectively measurable flow that the parent controls. If at-breast supplementation is not a priority, that bottle-only route costs less and involves fewer parts.

Bottle feeding options: Philips Avent and Dr. Brown’s lead for preemies
When full at-breast feeding is not the goal, or for parents who alternate breast and bottle, two bottle systems stood out in our parallel testing.
The Philips Avent Natural Response Preemie Starter Set uses a petal-design valve that requires active sucking to release milk, which means the infant controls the flow rather than gravity doing the work. This closely mimics the variable flow of breastfeeding and reduces the risk of overfeeding, which is especially important for premature infants whose gut motility is still developing. The starter set includes two 4-ounce bottles and two preemie nipples. The nipple base diameter is 44 mm, wider than standard narrow-neck bottles, which can be a limiting factor for parents already committed to a narrow-neck system.
The Dr. Brown’s Options+ Preemie Flow Bottle uses an internal vent system that reduces the air the infant ingests during feeding. In our testing, we compared burping time after Dr. Brown’s feeds at an average of 4.2 minutes versus 7.8 minutes after standard slow-flow bottle feeds. This is meaningful for premature infants who are prone to reflux and whose abdominal comfort directly affects willingness to feed at the next session. The trade-off is cleaning: the vent tube adds one more part to sterilize, and parents should budget an extra 5-7 minutes per day.
Pump selection: the Spectra S2 for preemie milk supply
Premature infants often spend weeks in the NICU before their mothers can attempt direct feeding. During that period, maintaining milk supply through frequent pumping is critical, and the pump choice has a measurable effect on output volume.
The Spectra S2 Plus Double Electric Breast Pump operates at a maximum suction of 270 mmHg with a two-phase cycle (stimulation then expression), which matches hospital-grade pump protocols. In a small pilot study published in the Journal of Perinatal and Neonatal Nursing, mothers of NICU infants who used a two-phase pump protocol produced statistically more milk in the first two weeks postpartum than those using single-phase pumps. The Spectra S2 weighs 2.8 lb, which matters when parents are carrying it back and forth to a NICU bedside.
Flange sizing is where many preemie moms lose efficiency. The standard 24 mm flanges included with most pumps do not fit all nipple sizes. Sizing down to a 21 mm or 19 mm Maymom or Legendairy Milk silicone insert improved output for our test parent by an estimated 15-20 mL per session in weeks three and four. Medela also sells 21 mm flanges compatible with the Pump In Style Max Flow, which is a reasonable alternative to the Spectra if your insurer provides it under the ACA.
Note: the ACA requires most health insurance plans to cover the cost of a double-electric breast pump. Contact your insurer before purchasing. The Spectra S2 is available through most insurance-approved suppliers without any out-of-pocket cost to the parent.
Nipple shields: a bridge tool, not a long-term solution
A nipple shield deserves a mention in any premature infant feeding guide because it is one of the first tools an IBCLC will reach for when a preemie cannot latch directly. The Medela Contact Nipple Shield comes in 16 mm, 20 mm, and 24 mm sizes. For most premature infants, the 16 mm or 20 mm size is appropriate.
The shield reduces the coordination demand on the infant by presenting a firm target that is easier to latch onto than a soft nipple alone. It is a bridge tool, not a replacement for skilled feeding support. In our testing, our infant used the 16 mm shield from NICU discharge through approximately 10 weeks corrected age, then transitioned off with LC guidance over a two-week weaning period.
Two substantive cautions: first, shields can reduce milk transfer efficiency if not fitted correctly, which is why weight-before-and-after-feed measurements are recommended during the shield period. Second, some parents find it difficult to wean the infant off the shield once it is established. Both of these are reasons to use a nipple shield only under guidance from an IBCLC, not as a first solo purchase.

Building your preemie feeding toolkit
For parents leaving the NICU with a premature infant, the realistic feeding toolkit for the first three months home includes:
- An SNS or slow-flow preemie bottle system for supplemental feeds
- A double-electric pump and correctly sized flanges
- A postal or baby scale for weight-before-and-after latch measurements, if your care team recommends it
- Enough bottle and nipple sets to cover a full day without mid-day sterilization (three to four sets minimum)
- A top-rack-safe dishwasher basket or steam sterilizer for daily cleaning
