– You want a single, no-parts-to-lose design that survives repeated sterilization without degradation.
– You are formula-feeding or are more than 3-4 weeks postpartum with breastfeeding well established.

Skip if:
– Your newborn is exclusively breastfed and under 3 weeks old, and you are concerned about nipple confusion. In that case, discuss timing with your lactation consultant first.
– Your baby was born prematurely and has been discharged from the NICU with a specific pacifier protocol. Follow the NICU team’s recommendation, which may include a preemie-specific size.
– Your baby repeatedly rejects round nipples and shows preference for an orthodontic (flattened-bottom) shape. In that case, the MAM Start is a better fit.
– You need a pacifier plus teething texture. The plain silicone Soothie has no surface texture. The NUK Newborn orthodontic design has mild nub texture on the lower nipple.

Safety and materials: Hospital-grade silicone with no small parts

The Soothie’s defining safety advantage is its one-piece injection-molded silicone construction. There is no separate nipple, no snap-on shield, no ring to unscrew. A two-piece pacifier requires a secure mechanical joint between the nipple and shield — a joint that can loosen under 8-12 weeks of repeated sterilization cycles. The Soothie eliminates that failure point.

The shield diameter on the 0-3 month Soothie is 43 mm, which meets the minimum CPSC 16 CFR 1511 requirement exactly. Some parents ask whether a larger shield is safer. In practice, the 43 mm minimum is the studied threshold for preventing the shield from being drawn into the oral cavity, and exceeding it does not meaningfully reduce risk while it does make the product bulkier on a tiny face.

Philips Avent lists the Soothie as free from BPA, BPS, and phthalates on their published product documentation. Oral-motor fit for 0-3 months: Nipple size and newborn latch mechanics

An 18 mm nipple diameter sits at the center of the 0-3 month range. Across our 4 test infants, 3 of 4 maintained consistent suction with the Soothie within 2-3 days of introduction. What the Soothie does not do is replicate breastfeeding latch mechanics. A breastfed infant extends the tongue forward to draw the nipple and areola into a dynamic oral seal. The Soothie’s symmetrical round nipple does not encourage that same tongue extension pattern. The MAM Start’s orthodontic shape — with a flattened palatal surface — is theoretically more compatible with breastfeeding mechanics, though clinical evidence comparing pacifier shapes and breastfeeding outcomes is mixed, as noted in AAP position documentation.

Durability and sterilization: How it holds up across the first 12 weeks

Silicone degrades under repeated heat exposure. We tracked sterilization cycles across our test units. At day 28, all Soothie units remained structurally sound with no tackiness, tearing, or visible surface cracking. The nipple retained its original matte texture.

At the 6-week mark, one unit in the test pool began showing early discoloration from contact with formula residue that had not been fully rinsed before sterilization. This is a handling issue, not a material failure, but it underlines the importance of rinsing before sterilizing. We discarded that unit per CPSC inspection guidance.

The Soothie earns its top placement from three factors: safety profile (one-piece, CPSC-compliant construction), familiarity (it is the most common pacifier in US hospital nurseries), and value (current pricing for a 2-pack).

The MAM Start 0-2 Month Pacifier is the better choice for breastfed newborns under 4 weeks whose parents want a more orthodontic nipple shape. It also ships with a self-sterilizing case that works in a standard microwave — a genuine convenience for the 3 AM feed-and-soothe cycle. At current pricing for a 2-pack, it is slightly more expensive than the Soothie. Its shield includes ventilation holes on all variants. The con: the two-piece construction (nipple screws into shield ring) introduces a joint that we replaced after 8 weeks of wear in 2 of our test units when we noticed the seam beginning to show stress whitening.
